A 25-year-old female presented with a complaint of pain and swelling in the right hand for 3 days after being bitten by a gecko 3 days ago. A soft tissue POCUS examination was performed on the dorsum of the hand over the region of the soft tissue swelling. The following image was obtained.

What is the most likely diagnosis?

A. Soft tissue abscess
B. Normal soft tissue
C. Cellulitis

Explanation

The B-mode image shows increased echogenicity of the soft tissue with evidence of anechoic fluid and giving the classic “Cobble-stone” appearance. The tissue architecture is preserved. Findings are suggestive of cellulitis. Always correlate clinically.
